fa میزان بروز زخم فشاری و عوامل مرتبط با آن در کودکان و نوزادان بستری در بیمارستان های آموزشی Incidence rate of pressure ulcers and associated factors among children and newborn hospitalized in teaching hospitals مراقبت‌هاي پرستاري Nursing Care پژوهشي Research زمینه و هدف: <a name="_Hlk121733489">زخمفشاری مشکلی جدی و قابل پیشبینی در نظام سلامت است</a>. مطالعه حاضر با هدف تعیین میزان بروز زخمفشاری و عوامل مرتبط با آن در کودکان و نوزادان انجام گرفته است. روش بررسی: در این مطالعه همگروهی 1025 نوزاد و کودک 0 تا 14 سال بستری در دو مراکز آموزشی درمانی دانشگاه علوم پزشکی کرمانشاه وارد مطالعه شدند. نمونهگیری به روش در دسترس از اسفند 1400 تا تیر 1401 انجام یافت. ابزارهای جمعآوری اطلاعات شامل پرسشنامههای اطلاعات، کومای گلاسکو، درجهبندی انجمن ملی مشاوره زخم‌فشاری و برادن Q بود. تحلیل دادهها با رگرسیون لجستیک تک متغیره و چند متغیره انجام یافت. یافته‌ها: میزان بروز زخمفشاری <a name="_Hlk134263382">97/4%</a>(51 نفر) بود. زخمهای فشاری از نظر شدت در 46 نفر(14/82%) درجه یک و بیش‌ترین محل بروز زخم در ناحیه دست 20 نفر(71/35%) بود. خطر نسبی زخمفشاری در کودکان و نوزادان با سطح هوشیاری کم‌تر از 14 (07/6=RR، 73/12–90/2: فاصله اطمینان 95%)، با مدت بستری بیش از 5 روز (08/1RR=، 12/1- 05/1: فاصله اطمینان 95%)، دارای مشکلات تنفسی (17/2=RR، 94/3–19/1: فاصله اطمینان 95%)، بیتحرک (46/6=RR، 13/12–44/3: فاصله اطمینان 95%)، دارای ماسک Ncpap (11/1=RR، 18/1–05/1: فاصله اطمینان 95%)، و پالساکسیمتر (07/6=RR، 10/1–03/1: فاصله اطمینان 95%)، به طور معناداری بالاتر بود. در آنالیز چند متغیره، مدت بستری بیش‌تر از 5 روز (89/4=RR، 40/10–29/2: فاصله اطمینان 95%)، (001/0>p) و بیتحرکی (49/2RR، 29/6–29/1: فاصله اطمینان 95%)، (01/0=p) با افزایش معنادار خطر ابتلا به PU همراه بود. نتیجه‌گیری: میزان بروز زخمفشاری در مطالعه حاضر نسبتاً پایین و کم‌تر از 5% بود. افزایش مدت بستری و بیتحرکی مهمترین و بعد از آن‌ها کاربرد ابزارهای پزشکی بهخصوص آنژیوکت و ماسک Ncpap، و ابتلا به بیماری‌های تنفسی سایر عوامل خطر مهم مرتبط با بروز زخم‌فشاری در کودکان و نوزادان شناسایی شدند.   Background & Aim: Pressure ulcers pose a serious and anticipated challenge within the health system. The present study aimed to determine the incidence of pressure ulcer and associated factors among children and newborns. Methods & Materials: In this cohort study a sample size of 1025 children aged 0 to 14 years hospitalized in two medical training centers affiliated with Kermanshah University of Medical Sciences, were included. The sampling method employed was convenience sampling, conducted between March and July 2022. The data collection tools utilized in this study included a demographic information questionnaire, the Glasgow Coma Scale, National Pressure Ulcer Counseling Association classification, and Braden Q Scale. The data was analyzed using both univariate and multivariate logistic regression analyses. Results: The incidence rate of pressure ulcers was found to be 4.97% (51 people). In terms of severity, the majority of pressure ulcers were categorized as first grade, accounting for 82.14% (46 people), with the hand area being the most frequently affected site of ulceration, representing 35.71% (20 people). The factors exhibited a significantly higher relative risk of pressure ulcers in children and infants included consciousness level of less than 14 (RR=6.07, 2.90-12.73: 95%CI), hospitalization duration of more than five days (RR=1.08, 1.05-1.12: 95%CI), presence of respiratory problems (RR=2.17, 1.19-3.94: 95%CI), immobility (RR=6.46, 3.44-12.13: 95% CI), utilization of NCPAP mask (RR=1.11, 1.05-1.18: 95% CI), and pulse oximeter (RR=6.07, 1.03-1.10: 95% CI). In the multivariate analysis, hospitalization duration of more than five days (RR=4.89, 2.29-10.40: 95% CI, P<0.001) and immobility (RR=2.49, 1.29-6.29: 95% CI, P=0.01) were found to be significantly associated with an increased risk of pressure ulcers. Conclusion: The incidence rate of pressure ulcers observed in the present study was relatively low, measuring below 5%. Notably, prolonged hospitalization duration and immobility were identified as the most important risk factors contributing to the development of pressure ulcers among children and infants.
